Rental Contract

Instructions:

• Download our Rental Agreement.
• Print two copies; fill out the top half of the contract through the credit card and payment authorization line. A current credit card must be on file to maintain an active rental contract, regardless of how payment is made.

• Level I instruments require a $50 deposit and are rented at $25 per month, plus tax.

• Level II instruments require a minimum deposit of $100 and are rented at $45 per month, plus tax. We reserve the right to increase the amount of the deposit if necessary. Instrument rental policies are shown at our web page Ellsworth Music: Retail Division.

• Call Ellsworth Music to reserve the instrument you wish to rent.

• For your convenience instruments may be shipped to you. Please call for details.
